The direction of CMS policy is legible: the agency is structurally incentivising the shift from episodic, encounter-based care to continuous, relationship-based care management. Platforms built to deliver that model at scale are positioned on the right side of this regulatory trajectory. The financial opportunity at the practice level is real and currently underleveraged. The majority of eligible Medicare beneficiaries remain unenrolled in CCM programmes despite eligibility and clinical evidence supporting their effectiveness. The gap between reimbursement available and reimbursement captured is a function of operational complexity that most practices cannot absorb with existing infrastructure.
