HealthRADAR: One Platform for Entire Patient Care Lifecycle Most healthcare technology is built to solve one problem. A remote monitoring device tracks vitals. A prescribing system speeds up documentation. A billing tool automates claims. Each of these is useful in isolation. But the chronic care challenge is not an isolated problem, and a collection of point solutions rarely adds up to a coherent system. What we built at Evolko starts from a different premise: that truly effective chronic care management requires a single, integrated workflow that spans the entire patient journey, before the appointment, during the appointment, and after it. Let us walk you through what that actually looks like in practice, because we think the architecture of HealthRADAR is best understood as a sequence rather than a feature list. Before a patient sits down with their physician, our Robotic Triage system has already done significant work. Patients initiate a triage either through the Evolko mobile application when booking a remote consultation, or through a tablet-based interface used by a physician assistant in the clinic. System gathers structured medical history aligned to the patient's specific condition and specialty context, a clinically organized pre-screening that builds a disease-specific profile. This profile reaches the physician before the consultation begins, meaning the appointment opens with context rather than discovery. Beyond the primary condition profile, Robotic Triage also generates a non-communicable disease risk assessment, screening for undiagnosed conditions that the primary visit reason might not surface. In practice, this means conditions like hypertension or pre-diabetes that might otherwise go undetected for years are identified early, when intervention is still relatively straightforward.  During the consultation itself, physicians work within Adaptive RxPad, a prescription and treatment planning interface that is specifically designed to conform to individual physician style rather than imposing a standardized workflow. Some have established protocols they apply reliably to certain patient profiles. The RxPad learns these patterns over time and adapts accordingly. The Protocol feature allows a complete, specialty-specific treatment plan to be initiated with a single click, not a generic template, but a plan built from clinical knowledge base content developed in collaboration with specialist physicians. Everything generated through RxPad is automatically saved to the patient's lifetime electronic medical record. After the appointment, HealthRADAR monitoring application takes over. Patients receive structured, condition-specific prompts that gather the data most relevant to their care team, blood pressure readings, glucose levels, symptom reports, medication adherence indicators. The application sends automated reminders, health tips calibrated to patient's profile and motivational content that supports sustained engagement. Roughly 60 % of the patients currently using our platform are over 65 year old, and the engagement rates among this demographic consistently exceed expectations. The data flows in real time to our nursing team and to the physician's dashboard, where clinical flags surface automatically when reported values suggest deteriorating status or approaching complications. Physicians and nurses can respond via secured messaging within the application, enabling timely clinical guidance without requiring the patient to present in person.  Underlying all of this is our Revenue Cycle Management module, Evolko RCM, which handles the billing and documentation complexity that has historically made Chronic Care Management programs operationally daunting for practices. Every patient interaction is logged automatically against the relevant CPT codes. Our certified medical coding team achieves a clean claim rate of ninety-six percent. A dedicated RCM account manager provides transparent reporting on financial performance and handles the documentation requirements that CMS mandates for CCM reimbursement. The result is that practices receive the recurring monthly revenue that their CCM program generates without absorbing the administrative burden of managing it.
