The monthly twenty-minute minimum sounds modest, but its cumulative effect is substantial. A patient receiving consistent monthly clinical contact &mdash; medication review, symptom check-in, care plan adjustment &mdash; is fundamentally better managed than one navigating their conditions alone between twice-yearly appointments. Patients enrolled in structured CCM programmes show measurably better medication adherence, earlier identification of worsening conditions, and significantly lower rates of preventable hospitalisation.
