Why We Started Evolko, the Problem That Would Not Leave Us Alone
There is a number that we could not stop thinking about when we first started building Evolko. The figure, five to nine percent annual growth in unmet demand for physician care. Sounds abstract until you understand what it actually describes. It describes patients with diabetes who see their doctor twice a year and spend the other three hundred and sixty-three days making decisions about their health without any professional guidance. It describes people with heart failure being readmitted to hospitals for complications that a phone call, a blood pressure reading, or a medication reminder might have prevented. It describes an entire population of people, tens of millions of Americans, living with conditions that require daily management but receiving episodic care at best.
We had worked in and around healthcare technology for long enough to see both sides of this problem clearly. We understood that physicians were not failing their patients out of indifference. They were operating within the constraints of a system that had never been redesigned to support chronic disease at the scale we now face. A fifteen-minute appointment every few months is simply not adequate care for a patient managing diabetes, hypertension, and early-stage chronic kidney disease simultaneously. It was never going to be. And yet for decades, that had been the standard.
What we believed and what the evidence consistently supported was that the solution was not more physicians. There are not enough, and training more takes a generation. The solution was infrastructure: technology and human expertise working together to extend the reach of the care that physicians were already providing. The technology to monitor patients between appointments, to gather structured clinical information before visits, to automate the documentation burden that was consuming enormous amounts of physician time. And the human expertise specifically, the expertise of licensed nurses whose capacity for patient support, education, and monitoring was dramatically underutilized by a system that had never found a scalable way to deploy it.
That was the founding insight behind Evolko. Not a specific feature or a particular piece of software, but a conviction that chronic care management could be transformed if we built the right platform around the right clinical model. A platform that connected patients, doctors, and nurses in a way that made the work sustainable for all three financially sustainable for practices, clinically effective for patients, and professionally meaningful for the nurses at the center of the daily monitoring work.
HealthRADAR, the platform we built, reflects every dimension of that original conviction. The Robotic Triage module means that a physician's first interaction with a patient in a consultation is informed, not exploratory the AI has already built a clinical profile, flagged relevant risk factors, and surfaced potential undiagnosed conditions that might otherwise have gone undetected. The Adaptive RxPad means that the prescription and treatment planning process adapts to each physician's style rather than forcing them to adapt to the software. And the monitoring application means that after the appointment ends, the patient is not left alone with a care plan they may or may not follow, they have reminders, health tips, and direct access to nursing support, all of it feeding structured data back to the physician for timely review.
We think about the founder of a small internal medicine practice in the midwest who told us that before Evolko, she had known for years that her Medicare patients qualified for CCM reimbursement but had never pursued it because the operational complexity felt impossible with her existing staff. Six months after onboarding with us, her practice was generating over eight thousand dollars per month in new CCM revenue and her patients were, by her own assessment, better managed than they had ever been. Before a patient sits down with their doctor, our Robotic Triage technology has already built a disease-specific clinical profile, gathering structured medical history, surfacing relevant risk factors, and even screening for undiagnosed non-communicable diseases that might otherwise go undetected for years. During the consultation, our Adaptive RxPad learns each physician's prescribing style, enabling treatment protocols to be set in a single click. And after the appointment, our monitoring app keeps patients engaged with their care plan, reporting symptoms, receiving reminders, and staying connected to a team of licensed nurses who monitor their status around the clock.
This is not theoretical.
